The Science of Aging: What Actually Slows It Down

Longevity has become one of the most crowded corners of the wellness industry, with supplement stacks, cold plunges, and pricey “biohacks” promising to add years to your life. A new analysis published in Nature Medicine cuts through the noise by asking a simple question: when researchers actually measure biological aging, which interventions hold up?

The answer, according to the Yale-led review, is both reassuring and humbling. The habits your doctor has recommended for decades are, so far, the ones with the strongest evidence. Many of the flashier interventions dominating social media feeds are not.

The Study Behind the Headlines

Researchers led by Raghav Sehgal, PhD, at the Yale School of Medicine analyzed 51 longitudinal intervention studies and evaluated their effects on more than 110 DNA methylation biomarkers linked to aging. These biomarkers, sometimes called “epigenetic clocks,” estimate biological age by measuring chemical tags on DNA that change in predictable patterns as we grow older.

The team then compared how various diets, exercise programs, medications, and supplements shifted these clocks across 16 well-validated aging biomarkers. The Nature Medicine paper represents one of the most comprehensive syntheses to date of what actually appears to influence biological aging in humans.

What the Evidence Supports

Mediterranean and Lower-Carb Eating Patterns

Diet consistently showed the clearest signal among lifestyle interventions. Mediterranean-style eating, low-carbohydrate diets, and low-fat diets each produced measurable shifts in epigenetic aging markers. The Mediterranean pattern — rich in vegetables, legumes, olive oil, nuts, fish, and whole grains — has the deepest research base overall, and it aligns with earlier findings from the National Institutes of Health linking this pattern to lower cardiovascular and cognitive risk.

The takeaway is not that one specific diet is uniquely magical, but that dietary patterns emphasizing whole plants, healthy fats, and minimal ultra-processed foods appear to influence biology at the molecular level, not just the waistline.

Regular Physical Activity

Exercise remained one of the most durable interventions across the review. Aerobic activity and resistance training both showed consistent effects on aging biomarkers, echoing decades of research linking physical activity to lower risk of heart disease, type 2 diabetes, dementia, and all-cause mortality. Global public health bodies, including the World Health Organization, recommend at least 150 to 300 minutes of moderate activity per week for adults, along with strength training at least twice weekly.

Certain Prescription Medications

Some of the strongest and most consistent effects across all 16 epigenetic clocks came from three classes of medication:

  • Metformin, long used to manage type 2 diabetes, has been studied for years as a possible longevity agent and is currently being evaluated in the ongoing TAME (Targeting Aging with Metformin) trial.
  • Semaglutide, a GLP-1 receptor agonist used for diabetes and weight management, showed effects on multiple aging markers.
  • Anti-TNF therapies, which suppress inflammatory signaling and are used for autoimmune conditions, also moved biomarkers.

These findings are intriguing but not a green light for healthy adults to seek prescriptions off-label. Each of these drugs has meaningful side-effect profiles and is currently approved for specific medical conditions. Any decision to use them requires a qualified prescriber.

What Did Not Move the Needle

The review found that most over-the-counter supplements — the category dominating longevity marketing — showed limited and inconsistent effects on aging biomarkers. “The evidence for supplements was limited and inconsistent,” Sehgal noted in coverage of the study.

This aligns with earlier reviews from the NIH Office of Dietary Supplements and the U.S. Preventive Services Task Force, which have consistently found insufficient evidence that multivitamins or most individual supplements meaningfully extend healthy lifespan in well-nourished adults. Popular anti-aging compounds like NMN, NR, and resveratrol have shown some promise in animal studies but have yet to demonstrate consistent, meaningful effects in human trials.

How to Read Epigenetic Clock Data

Epigenetic clocks are a useful research tool, but the researchers were careful to note their limits. A shift in a biomarker is not the same as proof that someone will actually live longer or healthier.

“Biomarkers are useful because they may let us see biological changes over months or years, rather than having to wait decades for outcomes like disease or death,” Sehgal explained. In other words, these clocks are best understood as reasonable proxies that help researchers screen interventions faster — not as scoreboards for personal longevity.

Consumers should be especially cautious about direct-to-consumer “biological age” tests marketed to healthy adults. Results vary considerably between clocks, day-to-day variability is significant, and no professional medical body currently recommends them for routine use.

The Broader Longevity Picture

The Yale review focused on interventions tested in trials, but decades of population research point to a wider set of habits associated with longer, healthier lives. According to the Centers for Disease Control and Prevention and multiple large cohort studies, the biggest levers include:

  • Not smoking — still the single most impactful modifiable factor for life expectancy.
  • Consistent, sufficient sleep — typically 7 to 9 hours for adults, with regular timing.
  • Maintaining a healthy body composition, particularly limiting excess visceral fat.
  • Managing blood pressure, cholesterol, and blood sugar through routine care.
  • Strong social connections, which recent research from the U.S. Surgeon General has linked to mortality risk on par with smoking.
  • Limiting alcohol, with newer guidance from bodies like the WHO indicating no consumption level is fully “safe.”

None of these are new or exotic. But their combined effect on healthy lifespan is difficult for any supplement or gadget to match.

What Researchers Actually Recommend

The bottom line from the Nature Medicine review echoes what most preventive-medicine physicians already advise: focus on the fundamentals. Regular physical activity, a mostly whole-food diet built around plants and healthy fats, adequate sleep, avoiding tobacco, moderating alcohol, and consistent management of any chronic conditions remain the interventions with the strongest evidence for slowing biological aging.

Newer areas of research — GLP-1 agonists, senolytic drugs, precision anti-inflammatory therapies — may reshape the picture in coming years. For now, the science suggests that the biggest longevity gains for most adults still come from doing the ordinary things well and consistently, rather than from any single supplement or trend.
