For years, observational studies have hinted at a link between higher vitamin D levels and better outcomes in colorectal cancer. A new Phase 3 randomized trial has finally put that question to a rigorous test — and the answer is more nuanced than either camp expected.
Published in JAMA in August 2026, the SOLARIS trial randomized 455 patients with metastatic colorectal cancer to receive either high-dose or standard-dose vitamin D3 alongside chemotherapy. The primary endpoint was disappointing for vitamin D advocates. But a subgroup finding is now driving fresh scientific interest.
What the SOLARIS Trial Tested
SOLARIS was designed to answer a practical question: if a patient is already receiving chemotherapy for advanced colorectal cancer, does adding pharmacologic doses of vitamin D3 slow the cancer or extend life?
- High-dose group: 8,000 IU of vitamin D3 daily as a loading dose, then 4,000 IU daily

Overall survival told a similar story: 120 deaths in the high-dose arm compared with 112 in the standard-dose arm, again not statistically significant. As lead investigator Dr. Kimmie Ng of Dana-Farber Cancer Institute noted, “the primary results did not show statistically significant differences” between the two treatment arms.
For clinicians hoping vitamin D would become a routine add-on for metastatic colorectal cancer, that headline result is a clear negative.
The Subgroup Signal: Left-Sided Tumors
The more intriguing finding sits inside a prespecified subgroup. In patients whose primary tumor originated on the left side of the colon — the descending colon, sigmoid, and rectum — high-dose vitamin D3 was associated with a median progression-free survival of 13.8 months, compared with 10.2 months in the standard-dose group.
Left- and right-sided colorectal cancers are increasingly understood as biologically distinct diseases. They differ in embryologic origin, microbiome exposure, mutation profiles, and response to certain targeted therapies. A signal that vitamin D3 may matter more on one side than the other is consistent with that emerging biology, but it needs to be treated as hypothesis-generating rather than practice-changing.
Subgroup analyses in oncology trials are notorious for producing findings that later fail to replicate. Until a confirmatory trial in left-sided disease is completed, the SOLARIS subgroup should shape research priorities — not personal supplement regimens.
Why Vitamin D Was Studied in the First Place
The rationale for testing vitamin D in colorectal cancer is grounded in decades of observational data. Analyses of large cohorts, including work from the Nurses’ Health Study and the Health Professionals Follow-up Study, have found that people with higher circulating levels of 25-hydroxyvitamin D — the standard blood marker of vitamin D status — tend to have lower colorectal cancer incidence and mortality.
Preclinical research adds mechanistic plausibility. Vitamin D receptors are expressed in colon epithelial cells, and laboratory studies suggest calcitriol (the active form of vitamin D) can influence cell proliferation, differentiation, and apoptosis in colorectal cancer models. The earlier SUNSHINE trial, published in JAMA in 2019, hinted at a progression-free survival benefit from high-dose vitamin D in metastatic colorectal cancer, which is what motivated the larger SOLARIS effort.
The pattern that has now emerged across randomized trials is familiar in nutrition science: observational associations do not always survive the transition to controlled interventional testing.
What This Means for Vitamin D and Overall Health
A negative cancer-treatment trial does not mean vitamin D is unimportant. According to the National Institutes of Health Office of Dietary Supplements, vitamin D is essential for calcium absorption, bone health, immune regulation, and neuromuscular function. Deficiency — often defined as a 25-hydroxyvitamin D level below 20 ng/mL — is associated with rickets in children, osteomalacia in adults, and increased risk of fractures.
The Institute of Medicine’s recommended dietary allowance for most adults is 600 IU per day (800 IU for adults over 70), with an upper intake level of 4,000 IU per day for adults. The very high loading doses used in trials like SOLARIS are far above what routine supplementation would deliver and are not appropriate for self-directed use.
People with limited sun exposure, darker skin, obesity, malabsorption conditions, or older age are more likely to have low vitamin D status. A simple 25-hydroxyvitamin D blood test can identify deficiency, and a healthcare provider can recommend an appropriate correction strategy.
The Bigger Picture in Colorectal Cancer
Colorectal cancer remains the second leading cause of cancer death in the United States, according to the American Cancer Society, though incidence in adults over 50 has been declining thanks to screening. Alarmingly, incidence in adults under 50 is rising — a trend that has prompted the U.S. Preventive Services Task Force to lower the recommended screening start age to 45 for average-risk adults.
The most reliably impactful moves for reducing colorectal cancer risk remain evidence-based lifestyle and screening measures: not smoking, limiting alcohol and processed red meat, maintaining a healthy weight, staying physically active, and completing recommended screening on schedule. Vitamin D adequacy fits within a broader nutritional pattern that supports overall health, but the SOLARIS trial argues against high-dose supplementation as a therapy for established metastatic disease.
What to Watch Next
Researchers are now weighing whether to launch a dedicated trial in left-sided colorectal cancer to test whether the SOLARIS subgroup signal replicates. Parallel work is exploring how tumor sidedness interacts with the microbiome, immune features, and vitamin D receptor expression — questions that could eventually explain why the biology may differ.
For patients currently being treated for colorectal cancer, the takeaway is that decisions about vitamin D dosing should be individualized in consultation with the oncology team, not driven by headlines or subgroup analyses.
