Waist Circumference vs BMI: What Better Predicts Health Risk

For decades, body mass index (BMI) has been the default number clinicians use
to gauge whether a patient is at a healthy weight. But a growing body of
research suggests that a simple tape measure around the waist may be a more
useful signal of long-term health risk than the ratio of weight to height.

Studies from the National Institutes of Health (NIH), the American Heart
Association (AHA), and large European cohorts indicate that where fat is stored
— particularly around the abdomen — matters more for metabolic and
cardiovascular risk than total body weight. Here is what current evidence
shows, and how to interpret the numbers.

Why BMI Has Limits

BMI is calculated by dividing weight in kilograms by height in meters
squared. It is inexpensive, quick, and correlates reasonably well with body
fat at a population level. But BMI does not distinguish between muscle and
fat, and it says nothing about where fat is stored in the body. Two people
with the same BMI can have very different metabolic profiles.

Researchers have long recognized a phenomenon sometimes called
“thin outside, fat inside” — individuals with a
normal-range BMI who nonetheless carry significant visceral fat around
their organs. A 2023 Annals of Internal Medicine analysis of more
than 40,000 U.S. adults found that BMI misclassified body fat status in a
substantial share of participants, particularly older adults and women.

What BMI Misses

  • Muscle mass (athletes often register as “overweight”)
  • Bone density and frame size
  • Fat distribution (subcutaneous vs. visceral)
  • Ethnic differences in body composition

Why Waist Circumference Matters

Waist circumference is a direct proxy for abdominal fat, and abdominal
fat — especially visceral adipose tissue surrounding the liver,
pancreas, and intestines — is metabolically active. It releases
inflammatory signaling molecules and free fatty acids that promote insulin
resistance, elevated blood pressure, and abnormal cholesterol patterns.

A landmark meta-analysis published in BMJ pooling data from
more than 2.5 million participants across 72 studies found that waist
circumference, waist-to-hip ratio, and waist-to-height ratio were each more
strongly associated with all-cause mortality than BMI. A 2024 European
cohort study of adults followed for more than a decade reported similar
findings: waist-to-height ratio outperformed BMI as a predictor of
cardiovascular events and premature death.

Current Thresholds

The National Heart, Lung, and Blood Institute (NHLBI) and the World
Health Organization (WHO) use these general cutoffs for adults of European
descent:

  • Men: waist circumference above 40 inches
    (102 cm) signals increased cardiometabolic risk.
  • Women: waist circumference above 35 inches
    (88 cm) signals increased cardiometabolic risk.

WHO and the International Diabetes Federation recommend lower thresholds
for South Asian, Chinese, and Japanese populations, who tend to develop
metabolic complications at smaller waist sizes. For South Asian men, the
suggested cutoff is roughly 35 inches (90 cm); for women, roughly 31 inches
(80 cm).

The Waist-to-Height Ratio: A Simpler Rule

Some researchers argue that waist-to-height ratio — waist
circumference divided by height — is the most practical single
measurement. The guideline that emerged from a large 2010 meta-analysis in
Obesity Reviews is often summarized as: keep your waist
less than half your height
. A ratio at or above 0.5 has been
linked in multiple studies to elevated risk of type 2 diabetes,
hypertension, and cardiovascular disease.

The United Kingdom’s National Institute for Health and Care
Excellence (NICE) updated its guidance in 2022 to recommend that adults
with a BMI under 35 also measure waist-to-height ratio as part of routine
risk assessment. NICE cited evidence that the ratio identifies
central-adiposity risk that BMI alone can miss.

What the Research Suggests About Disease Risk

Beyond cardiovascular disease, elevated waist circumference has been
linked in observational research to:

  • Type 2 diabetes: Visceral fat is strongly associated
    with insulin resistance. A prospective analysis in Diabetologia
    found waist circumference to be a stronger predictor of incident
    diabetes than BMI.
  • Certain cancers: The World Cancer Research Fund
    cites central adiposity as a probable risk factor for colorectal,
    pancreatic, endometrial, and postmenopausal breast cancers.
  • Non-alcoholic fatty liver disease: Larger waist
    size is a well-established correlate of hepatic steatosis, even in
    people whose BMI is in the normal range.
  • All-cause mortality: Multiple pooled analyses
    report a graded increase in death risk with rising waist
    circumference, independent of BMI.

How to Measure Your Waist Accurately

The CDC recommends the following steps for a consistent measurement:

  1. Stand upright and remove clothing over the abdomen.
  2. Locate the top of the hip bone and the bottom of the ribs.
  3. Wrap a soft tape measure horizontally around the midpoint,
    typically at the level of the navel.
  4. Exhale gently and take the reading without pulling the tape tight.
  5. Measure at the same time of day for consistency.

What This Means in Practice

BMI is not going away — it remains a useful population-level
screen and is embedded in clinical workflows worldwide. But research
suggests that adding a simple waist measurement, or calculating
waist-to-height ratio, can flag risk that BMI overlooks. That extra
signal is especially relevant for people who fall in the “normal
BMI” range but carry disproportionate abdominal fat.

Lifestyle strategies studied for reducing visceral fat include regular
aerobic and resistance exercise, diets emphasizing fiber and whole foods
(such as the Mediterranean pattern), adequate sleep, and limiting
ultra-processed foods and sugar-sweetened beverages. Studies indicate
that even modest reductions in waist circumference are associated with
improvements in blood pressure, blood sugar, and lipid markers.

Anyone with a waist measurement above the recommended threshold, a
family history of cardiometabolic disease, or concerns about body
